Our Care Advocates will sit down with you and your loved one and create a profile of “wants and needs” in order to develop and implement an action plan.
We will visit, interview and submit an objective and unbiased report of up to three nursing homes in your chosen geographic location. We will also observe, evaluate and report on accreditation and government reports, cleanliness, staff to patient ratio, cost and level of care.
If you need to apply for Medicaid benefits, we can coordinate and manage that process as well.
